Valid for one (1) calendar year from the date of enrollment. The Lighthouse Smile Club is not available to individuals who are enrolled in private dental insurance plans and may not be used in conjunction with such coverage. Individuals enrolled in New York State Medicaid or other New York State–subsidized dental plans are eligible to participate. Payment in full is due upon signing up for the plan. All fees are nonrefundable.
